On a 12 string tuned down a semi-tone, and a capo added at
the 1st fret to bring it back to standard pitch.

Play the G chords using middle, ring and little finger.
and then the C/G is formed by hammering-on the extra notes
on 2nd and 4th strings using the index and middle fingers,
making sure not to play the 5th string.
This is the way John Denver played it, but you could just
use a standard C chord instead, if you prefer to have a
C note in the bass.
